Principles of Breast Reduction Surgery the breast is made up skin, glandular tissue, fat and the nipple - areola

and repositioning the site of the nipple. The nipple is taken to its new position by a " pedicle " or block
of tissue with its own blood supply and this tissue may be based inferiorly, superiorly or from both sides.
While there are many types of operations for breast reduction, probably the procedure done most often
is an " inferior pedicle " type, as first described by Robbins, where the nipple areola complex is lifted
to its new site on an inferior pedicle. This is a safe, time proven method, which gives consistent results. The scars
from the surgery include a circular scar around the areola (the colored skin around the nipple). Another incision
is made in the form of an inverted " T " shaped scar with the vertical limb of the "T" running
downwards on the breast. The horizontal limb of the incision would be running across the lower part of the breast
and on to the chest towards the midline and the outer part of the chest wall.
Other methods with superior pedicles or from the sides have similar scars. A method gaining popularity
is the "Vertical Mammoplasty". In this procedure, the circular areola scar is similar to that of the inferior
pedicle procedure but the inverted vertical limb of the " T " runs downwards and curves outwards but stays
on the breast itself and there is no horizontal scar. This does produce a much shorter scar. In addition, the shape
of the reduced breast seems to be better due to some of the details of the actual procedure. However, the disadvantages
of the method are the shape of the breast for the first few weeks and the need for some revision surgery in those
patients needing larger reductions.

breast size while enhancing the appearance of your breast.

Reduction with Liposuction
The breast is made up of a number of tissues including glandular tissue, which is fibrous and not suitable for liposuction

breast reduction surgery is not only about volume reduction but also about creation of a new breast mound. Surgery,

with the development of pulsed Carbon Dioxide Lasers for skin rejuvenation. A laser (Light Amplification of Stimulated
Emission of Radiation) is light produced by various substances and which is capable converting into heat and energy
to destroy the target cells. Each laser has a specific wavelength on which depends the depth of skin penetration
of the laser and the color of the tissue it affects. Lasers, which are visible to the human eye, have shorter wavelength
and are used for the many conditions mentioned earlier. Lasers with longer wavelengths are invisible to the human
eye and Carbon Dioxide Laser and Erbium Laser are examples of this second type. These lasers are attracted to water
within cells and can remove the superficial layer of skin. In addition, the Carbon Dioxide Laser is also capable
of causing collagen contraction in the dermis (deeper two-thirds of skin). This combined effect (on the epidermis
and dermis) and with subsequent new skin regeneration causes facial rejuvenation with improvement in the appearance
and tone of skin, literally a face-lift without a knife and with new, tauter skin rather than the same skin being
tightened. The short wavelength, visible usually need a course of 4-6 or more treatments, with little or no anesthesia,
at intervals of between 2 weeks to 2 months and with little post treatment care. The long wavelength, invisible lasers
need a single course of treatment under some type of anesthesia and the post treatment care is very involved.

(broad spectrum pulsed light source) takes about 4 or more sessions, approximately 6 to 8 weeks apart. The treatment

the superficial part of skin to let the dye leach out as well as to encourage scavenger cells in the body to absorb
some of the dye. Dressings are needed for about 10 to 14 days after each treatment. Professionally done tattoos
respond better and the darker the color the easier the treatment.

Liposuction
A process, in which fat is removed from various parts of the body, using anesthetic tumescent solution and suction
devices has many uses. Ideally, liposuction is used for the removal of fat deposits beneath the skin where the skin
tone is good. A good example of this is the fat in the upper and outer part of the thighs ("saddle bags ")
in women. Over the years, there have been changes made which have expanded the uses of these techniques. Liposuction
is almost always done with a tumescent technique, where a large amount of specially prepared fluid is injected into
the area to be treated. This serves as an anesthetic and also helps reduce bleeding. The suction is done with a specially
designed machine, which is capable of very high suction pressures. Alternatively, suction with hand held syringes
could be done. Ultrasonic liposuction is available but there is not yet enough evidence to show that it is safe in
the long term. The original technique was used mainly on young and healthy skin with good tone. The newer technique
of simultaneous superficial and deep Liposuction permits even skin with poor tone to be treated with liposuction
as the superficial suction causes considerable skin retraction. The thighs and the gluteus areas are the most common
areas treated in women, whereas the waist is very commonly treated in men.

The drooping of skin, muscle and fat in the neck occurs with the passing of time, but earlier in some than in others.
